Trade Partner Prequalification & Setup Form

A. General

All insurance policies shall remain in full force and effect throughout the duration of the Contract. Trade Partner shall not commence work until all required Certificates of Insurance and endorsements have been submitted to and approved by Contractor.

Insurance shall comply with the Contract Documents and Prime Contract. Please note that all insurance certificates should have the following Certificate Holder information:

In no event shall coverage be less than the following:

1. Workers’ Compensation Insurance

• Coverage A: Statutory Benefits
• Coverage B: Employer’s Liability
• Bodily Injury by Accident: $1,000,000 each accident
• Bodily Injury by Disease: $1,000,000 policy limit
• Bodily Injury by Disease: $1,000,000 each employee

Required Endorsements:

• Waiver of Subrogation
• U.S. Longshore and Harbor Workers’ Compensation Act Endorsement, where applicable

2. Commercial Automobile Liability Insurance

• Limit: $1,000,000 each accident, combined BI and PD
• Coverage for Any Auto or All Owned, Scheduled, Hired, and Non-Owned Autos

Additional Requirements:

• Contractor and Owner named as Additional Insureds
• Pollution Liability coverage required if hauling hazardous materials
• Motor Carrier Act Endorsement (MCS-90) when applicable

3. Commercial General Liability Insurance

(Occurrence Form Only)

• Each Occurrence: $1,000,000
• Personal / Advertising Injury: $1,000,000
• Products / Completed Operations Aggregate: $2,000,000
• General Aggregate (per project): $2,000,000
• Maximum Deductible: $25,000

Required Coverage and Endorsements:

1. Premises and Operations with no XCU exclusions
2. Products and Completed Operations
3. Blanket contractual liability covering all indemnity obligations
4. Broad Form Property Damage, including completed operations
5. Additional Insured status for Contractor, Owner, and others, for ongoing and completed operations through Florida statute of repose
6. Primary and non-contributory wording
7. Occurrence form only
8. General aggregate limits on a per-project basis
9. Waiver of Subrogation

b. DEDUCTIBLE MUST BE LISTED

4. Excess / Umbrella Liability Insurance MINIMUM

• Each Occurrence: $1,000,000
• Aggregate: $1,000,000

5. Errors & Omissions (Professional Liability) If Applicable

(Required when design or engineering services are provided)

• Limit: $1,000,000 per claim
• Must be maintained for five (5) years after project completion
• Retroactive date prior to commencement of professional services

6. Pollution Legal Liability Insurance

• Limit: $1,000,000 per occurrence or per claim

No mold, fungi, bacteria, or water intrusion exclusions permitted when scope includes water protection or containment. If claims-made, a five-year extended reporting period is required.

7. Other Insurance Requirements

1. Thirty (30) days written notice of cancellation or modification
2. Deductibles and SIRs are Subcontractor’s responsibility
3. Insurers rated A-VII or better by AM Best
4. Certificates and endorsements required prior to site access
5. Contractor may require higher limits based on risk
6. Subcontractor responsible for all rented equipment
7. EIFS work must not be excluded from coverage

B. Waiver of Subrogation – Property Damage

Contractor and Subcontractor waive all rights against each other, Owner, Architect/Engineer, and other subcontractors for damages covered by Builder’s Risk or property insurance, except rights to proceeds. Deductibles shall be borne by the benefiting Party.

The undersigned certifies that all information provided is true and complete and acknowledges that compliance with all licensing, safety, insurance, and contractual requirements is mandatory.
